Q11: Is Dr. Mudrak a Blue Cross Blue Shield provider?
A: Yes. Dr. Mudrak's office submits all applicable claims to BCBS for particpating patients, because she is a BCBS provider.
As of May 1, 2012, Dr Mudrak is a participating provider for the following BCBS networks:
BCBS platinum network; BCBS precision level 2 network; BCBS blue performance basic 1; BCBS accord network; BCBS value network; BCBS blue perform enhance level 1; BCBS PPO; Medicare Blue PPO.
Note: Dr. Mudrak is not a provider for every BCBS network. Furthermore, the networks change regularly, Dr. Mudrak's particpation may change without notice and/or the network your employer belongs to may change. Verification of your policy benefits (BCBS or otherwise) is a good idea if you are planning to use insurance.
Q12: What questions should I ask my insurance company when I call to ask about benefits?
A: Please note, that for insurance plans other than BCBS Dr. Mudrak, is an out-of-network provider.
Please ask the following questions for BCBS:
- Is acupuncture covered by my insurance?
- If acupuncture is covered are there any criteria? (If there are criteria, for example such as chronic pain, be sure to request written copy of the criteria.) If a referral is needed, be sure to understand their referral policy.
- Am I in a BCBS network covered by Dr. Laura Mudrak, NPI #1366594673?
- What is my effective date of coverage?
- Is my plan on a calendar year, or other year (for example, September 1 to August 31)?
- What is my deductible: per individual, and per family?
- Has my out-of-network deductible been met yet for the year?
- Do I have a co-payment? If so, what is it?
- Will I owe a co-insurance? (After my deductible has been yet, what percentage does my insurance pay?)
If you have any insurance other than BCBS:
- Do I have out-of-network benefits?
- What is my effective date of coverage?
- Is my plan on a calendar year, or other year (for example, September 1 to August 31)?
- For patients with any insurance besides BCBS: What is my out-of-network deductible: per individual, and per family?

Q15: Is Dr. Mudrak a ChiroCare provider?
A: No. For your information, Medica and Health Partners distinguish between "in-network" providers and "out-of-network" providers. "In-network" refers to the ChiroCare network. When you contact Medica or Health Partners, please ask whether you have "out-of-network" benefits.

Q16: Is Dr. Mudrak a Medicare provider?
A: Yes. Medicare will pay for chiropractic manipulation to the spine only. Medicare will not pay for other services when prescribed or performed by a chiropractor. These services include acupuncture, lab tests, x-rays, treatment therapies besides manipulation, nutritional supplements, and physical examinations.
